The Medicare GUIDE program is a Centers for Medicare and Medicaid Services (CMS) initiative providing comprehensive care navigation, 24/7 clinical access, and up to $2,500 per year in in-home respite reimbursement for eligible seniors with a dementia diagnosis. Cognihealth is a Medicare GUIDE program partner in San Diego County, working with Tembo Health and PocketRN.
